How It Works

Step 1
Provider Visit

A one-on-one visit with one of our specialized providers with vast experience in providing care to seniors. Assessing your needs and get your the right equipment you need at no cost to you.

Step 2
We Ship Your Devices

We provide you with easy-to-use devices like a glucometer, scale, or blood pressure cuff that work just like standard devices. However, these tools automatically send your health data directly to our medical team.

Step 3
Monitor Your Readings

Your device readings are monitored 24/7 by our medical team, who can share data with your Primary Care Doctor or loved ones. If readings fall outside safe limits, we’ll immediately contact you, your family, or emergency services.
